I filled it out. It could have used a "not sure" option, though, because I honestly don't know yet if I want to have kids.
(Though at age 30 in less than a month, if I haven't made up my mind yet, that's probably a "no" -- or maybe I just haven't found someone with whom I'd want to have a kid!)

I agree! As my sister has Down's Syndrome, I hope the majority of people answer this question "yes" -- Though sadly, I wouldn't be surprised if it gets more "no's." (not necessarily from folks on this board -- just from whoever takes the survey in general!)
